Hats available to purchase

COST: $45 Per PLAYER (Includes fee, cart and food on the course)
　　　 $5 for CCS ALUMNI ASSOCIATION (IF NOT ALREADY PAID)

HANDICAP-If you have an established handicap please include it.
If not, include your normal score for 9 and/or 18 holes

   Team—You may form your own teams or be paired up with others to form a 4-person team.　
                 All teams will be handicapped.
　
   Please fill in the form and send it along with a check for the correct amount to me ASAP.　
   We are looking to fill out a 15-team field.　Should we have more interested, we will then
   consider opening the competition to a wider field. Spouses are welcome to participate.
